This Privacy Policy describes the information collected through this website, and the Health Care Relief™ service (“Service”) offered through DecisionPoint Health Analytics, LLC, and how that information is used and shared. DecisionPoint Health Analytics, LLC, ("DPHA”, "we," "us," or "our"), and its affiliations with other service providers and referral partners may collect or receive the types of information described below in connection with your use of Service, including your subscription agreement (together, "Collected Information").
We reserve the right to modify this Privacy Policy at any time. All changes will be effective immediately upon posting to our website, and by using Service, you agree to those changes.
Information Security
DPHA and Service do not guarantee that loss, misuse, or alteration of Collected Information will not occur, but we have security measures in place to help protect against the loss, misuse, and alteration of information under our control, including Secure Socket Layers (SSL) technology, which is used to encrypt information passed between your browser and Service. The storage and communication of information can never be completely secure, so we do not guarantee that communications or other information that you submit or that are otherwise provided to us will be completely secure. If you become aware of any breach of Service security or this Privacy Policy, please notify us at info@decisionpointsvcs.com. If our security system is breached, we will notify you of the breach only if and to the extent required under applicable law.

Personally Identifiable Information
“Personally Identifiable Information” refers to information that, when used alone or with other relevant data, can identify you. We will collect any Personally Identifiable Information from or about you that you choose to provide to us or that may be provided to us through your health care provider, which may include, but is not limited to, your name, email address, mobile telephone number, birthdate, and physical address. Specifically, we collect your Personally Identifiable Information when you use Service, in receiving and fulfilling requests for information, and when you respond to communications from us (e.g., requests for additional information and surveys). If you provide your mobile telephone number to us, you consent to receiving text (SMS) messages from us at that telephone number. We will use your Personally Identifiable Information to process your requests, to communicate with you in connection with your subscription, and to provide you with information and communications that you request or that we believe are relevant or may be of interest to you, subject to applicable laws and the terms of this Privacy Policy. We may also share your Personally Identifiable Information with third parties, including health care providers (“Providers”) and provider directory, search, and scheduling services resources and partners we engage to provide services and support in connection with Service.
Protected Health Information
If you use Service to schedule an appointment directly with a health care provider or through one of our Service advocates or through one of our directory, search, and scheduling services resources and partners, we may provide some of the information you have provided to Service to the health care provider or to a directory, search, and scheduling services resource or partner. Unless specifically stated otherwise, references to "Collected Information" in this Privacy Policy do not include Protected Health Information ("PHI") as that term is defined under the Health Insurance Portability and Accountability Act of 1996 and any regulations promulgated thereunder (together, "HIPAA"). We may also collect information, including but not limited to PHI from your health care provider or other authorized third parties when they submit such information through Service.
Subscription Information
To subscribe to Service, we will collect your first and last name, street address, mobile telephone number, and email address.
Subscription Billing, Collection and Payment Information
When you make a payment for Service, your payment card and information is collected and stored by our payment processing partners, Recurly (https://recurly.com,) and Stripe (https://stripe.com). Our payment processing partners collect your voluntarily-provided information to process the payment. Such information is stored and governed by their respective terms of service and privacy policies as disclosed on their websites.
Clinical Care and Behavioral Care Appointment Information
When using Service to locate and schedule an appointment at a nearby Federally Qualified health Center (FQHC) or other provider offering free (charitable) care or fee discounts based on household income, or when using Service to locate and schedule an appointment at a nearby Certified Community Behavioral Health Center (CCBHC) or other provider offering free care or fee discounts based on household income (collectively referred to as “Charitable Provider”), we may provide some of the information you have provided to Service to the Charitable Provider. Any additional Personally Identifiable Information or Protected Health Information you choose to provide toa virtual provider (collectively referred to as "Virtual Provider(s)"), is solely between you and the Virtual Provider. Such information is stored and governed by their respective terms of service, privacy policies, consent forms, and HIPAA privacy forms.
Telemedicine, Telehealth, Tele-behavioral Health Virtual Appointment Information.
When using Service to access virtual services offered through affiliated virtual service providers, any Personally Identifiable Information or Protected Health Information you choose to provide to the Virtual Provider is solely between you and the Virtual Provider. Such information is stored and governed by their respective terms of service, privacy policies, consent forms, and HIPAA privacy forms.
Blood Work, Imaging Tests, Colonoscopies and Upper GI Appointment Information
When using Service to access providers offering cash-pay pricing for blood/lab work, imaging tests, colonoscopies, upper GIs and other surgical procedures, (collectively referred to as “Outpatient Care Provider”), we may provide some of the information you have provided to Service to the Outpatient Care Provider. Any additional Personally Identifiable Information or Protected Health Information you choose to provide to the Virtual Provider is solely between you and the Virtual Provider. Such information is stored and governed by their respective terms of service, privacy policies, consent forms, and HIPAA privacy forms.
Financial Assistance Offered by Non-profit Hospitals for Medically Necessary Care
When using Service to apply for financial assistance for medically necessary care at a non-profit hospital (“Hospital”), we may provide some of the information you have provided to Service to the Hospital. If you are completing the financial assistance yourself, any additional Personally Identifiable Information or Protected Health Information you choose to provide to the Hospital Provider is solely between you and the Hospital. Such information is stored and governed by their respective terms of service, privacy policies, consent forms, and HIPAA privacy forms. If you choose to grant authority to Service to act as your consultant and assigned agent for all matters and documents pertaining to your application for financial assistance, by granting this authority you consent to having your Protected Health Information ("PHI") disclosed to us as minimally necessary for the completion and submission of your financial assistance application.
Mailing List Information
As a subscriber to Service, you may receive periodic informational and promotional emails (“Mailers”) from us. You may opt out of receiving promotional emails from us at any time by following the unsubscribe instructions contained in the applicable email. Mailers may collect the Internet protocol ("IP") addresses assigned to your computer, your operating system type, and demographic information about you. Mailers also uses cookies and other tracking technologies to collect information about your online activity and uses web beacons to track behavior (for example, which recipients opened emails and when, which recipients clicked on links contained in those emails, and your browser and email client type). Mailers may combine this information with information about you obtained by searching the Internet (for example, your use of social media platforms). Mailers may use your information and share it with third parties and for purposes of promoting its services, communicating with you, to comply with legal requirements, and to protect its legal rights. Our mailing list is managed using Zoho (https://www.zoho.com/) Such information is stored and governed by Zoho’s terms of service and privacy policy as disclosed on their website.

Service automatically uses cookies to track Service use and to facilitate and enhance the user experience on the Service. "Cookies" are small pieces of information that are stored on your computer's hard drive by your browser. Service may use both cookies that we implement, and cookies implemented by our service affiliates and other third parties. We may share information that you provide us or that we may collect about your use of Service with those platforms and that information will be subject to their privacy policies. Service may also use the advertising services and networks offered by those social media platforms to deliver advertising content. If you do not wish a social media platform to collect information relating to you via Service, we suggest that you: (i) hide social media platform plug-ins using an ad blocker; (ii) always log off completely from the social media platform before visiting other web sites and delete all related cookies; and, (iii) where the option is available, disable the advertising services and networks operated by the social media platform.
