Complete the Subcription Form
Click the "Subscribe" button below.
- Complete your Subscription Agreement and click the "Submit" button.
- Click "Subscribe" in the navigation bar to return here.
- Go to Step 3 of 4: "Submit Payment".
Step 3 of 4: Submit Payment
Activate subscription and submit payment
Select your subscription and click the button.
- Enter your contact information.
- Provide billing information.
- Submit. This will initiate automatic monthly charges to your credit/debit card over the 12-month term of your subscription.
- Click "Subscribe" in the navigation bar to return here.
- Go to Step 4 of 4.
Step 4 of 4: Breathe a Sigh of Relief!
Once your subscription is active, you'll receive a "Welcome to Health Care Relief" email with the following attachments:
- A PDF copy of your Subscription Agreement.
- One-pager on how to download your subscription eCard to your phone (optional)
- Instructions on how and when to use your subscription.
